Endless Journey provides an interdisciplinary approach to care by utilizing a team comprised of a Medical Director, RN Case Manager, Hospice Aide, Social Worker, Chaplain, Bereavement Coordinator, Registered Dietitian, Volunteer Coordinator, and alternative-care professionals.
By working together, our staff creates an individualized Plan of Care for every client based on their unique needs and wants.
Our Difference
Endless Journey is a community based hospice service providing care to eligible clients within the home or facility setting. The service is initiated by a referral from the physician, family member or client when it has been determined that the illness is terminal, and aggressive treatment would be ineffective, no longer viable, or has been declined by the client.
