The Endless Journey Process
Endless Journey is a community based hospice service providing care to eligible clients within the home or facility setting. The service is initiated by a referral from the physician, family member or client when it has been determined that the illness is terminal, and aggressive treatment would be ineffective, no longer viable, or has been declined by the client.
The Care Team
The client is then assessed for eligibility and if so determined, physician orders are established and the client may be admitted to hospice care.
The service is comprised of an Interdisciplinary Approach that includes:
- Medical Director
- Manager of Clinical Practice
- RN Case Manager
- Hospice Aide
- Medical Social Worker
- Chaplain
- Bereavement coordinator
- Registered Dietitian
- Volunteer Manager
A Multi-Disciplinary Approach
All disciplines work within their scope of practice and per Medicare/Medicaid guidelines. We do not assume the role of primary caregiver, instead provide a supplemental support service that manages the terminal illness with priority to pain control and quality of life, per direction of the client’s physician and the hospice Medical Director. Endless Journey recognizes the benefits of non-pharmacological interventions and a holistic approach to end of life care. Additional services are offered to nourish clients’ minds, bodies and spirits including but not limited to: massage therapy, acupuncture, dietary consultations/alterations, Dentistry, Chiropractic services, reiki, healing touch, aromatherapy, guided meditation, pet and music therapy etc.
Endless Journey was founded by a Registered Nurse with 10+ years of hospice experience and a passion for client choice. She pre-selected a team of experienced hospice professionals to develop a business plan with the focus of education and the restoration of dignity to the dying process. Hospice pre-dates the medical model and regulated eligibility. Families have been surrounding their loved ones in their homes with the goal of comfort as they prepared for end of life for decades. The origin is inspiring and paramount to the success of Endless Journey. The Endless Journey hospice experience will encompass the holistic philosophy in that the client and their family will be nourished physically, emotionally and spiritually as they prepare for end of life and beyond, with respect for client choice. Only in this way can Endless Journey be a true success.
