Social Work

Social Work as a support service is dependent upon client need. The Social Worker will complete an initial assessment to determine an appropriate plan of care. The purpose is to identify challenges that may impede the client’s vision for their end of life transition.

This includes but is not limited to:

  • Assist during the admission process
  • Facilitate care plan meetings
  • Connecting clients and families to resources to over come financial hardship.
  • Help clients and their families navigate planning for end-of-life care.
  • Social workers not only provide emotional guidance and practical help but also medical care coordination that can improve a patients quality of life throughout their journey.
  • Social work tasks: FMLA, medical necessity letters, Power of Attorney, funeral arrangements, grievances, and referrals to private caregivers.

An on-call psychosocial team member is available after hours to assist with crisis management and will communicate all involvement in client care with the assigned staff member. Our goal is to advocate for client choice and to support the client’s journey.

Spiritual Care

Our idea of a holistic experience includes the services of a chaplain to provide spiritual support as directed by the client and family. Religion is respected; however, all spirituality is supported. This service is optional and can supplement the client’s current practices. The chaplain will help coordinate support services with local congregations, parishes, synagogues, etc. as directed by the client and family. The chaplain is available to facilitate memorial services, funerals, and celebration of life events per request.
